Monday to Friday, 8.30am to 4.30pm (closed Public Holidays)

The City of Swan provides support services for frail aged, people with disabilities and people diagnosed with dementia and their carers. This service, named Swan Community Care Services, is funded by the Home and Community Care Program (a joint State and Federal initiative) and managed by the City of Swan. The aim of the service is to provide practical help and support that enables people to remain independent, to improve their quality of life and delay entry into residential care.
